Dr. Samuel Crutcher

Samuel John Crutcher, M.D., is a board-certified family medicine physician, a Lebanon native, and co-owner of Family Medical Associates & Pediatrics. He has been caring for patients at Family Medical since 2010, providing comprehensive primary care to individuals and families throughout Wilson County. His clinical experience also includes inpatient and nursing home medicine, giving him a broad perspective on caring for patients across different stages of life and healthcare settings.

Dr. Crutcher earned his medical degree from the University of Tennessee Health Science Center College of Medicine in Memphis and completed his Family Medicine residency at Palmetto Health Richland in Columbia, South Carolina. Following residency, he completed a fellowship in Quality Improvement through Palmetto Health Richland and the University of South Carolina.

Throughout his career, Dr. Crutcher has combined patient care with physician leadership and a commitment to improving healthcare quality. In addition to his work with Family Medical Associates and Vanderbilt Wilson County, he serves as Medical Director for Amedisys Home Health in Lebanon and has previously held medical leadership and academic teaching positions.

Dr. Crutcher has been certified by the American Board of Family Medicine since 2009 and is a member of the American Academy of Family Physicians. His dedication to his patients and community was recognized when he was voted Best Primary Care Provider in Wilson County in 2025 and again honored as a Top 3 Finalist for Best Primary Care Provider in Wilson County in 2026.

A native of Lebanon, Dr. Crutcher is proud to care for patients and families in the community he calls home.
