Stormiee Eldred, FNP-BC

Stormiee Eldred, FNP, is an experienced Family Nurse Practitioner with an extensive background in geriatric medicine, long-term care, primary care, and complex chronic disease management. She joined Family Medical Associates & Pediatrics in January 2022 and primarily provides medical care to patients residing in long-term care, skilled nursing, and assisted living communities throughout the region.

In her role with Family Medical, Stormiee works closely with facility nursing staff, patients, families, and other members of the healthcare team to provide ongoing medical management for older adults. Her experience includes caring for patients with multiple chronic conditions, evaluating acute changes in health, reviewing and interpreting laboratory and diagnostic testing, medication management, wound and infectious disease concerns, pain management, and coordinating care across healthcare settings. Her prior long-term care experience included responsibility for a census of approximately 80 residents and collaboration with physicians, nursing, dietary, social services, wound care, psychiatric services, and patients' families.

Stormiee’s healthcare career began well before becoming a Nurse Practitioner. She spent approximately nine years as a Registered Respiratory Therapist at Vanderbilt Children’s Hospital, working in pediatric critical care and emergency medicine. She later returned to school to become a Registered Nurse and spent three years caring for critically ill children in Vanderbilt Children’s Hospital’s Pediatric Critical Care Unit. These experiences provided a strong foundation in clinical assessment, complex patient management, patient and family education, and interdisciplinary care.

Stormiee earned a Bachelor of Science in Cardiopulmonary Care Science from Tennessee State University, completed her nursing education at Cumberland University, and earned her Master of Science in Nursing with a family focus from Tennessee Tech University. She is also ANCC certified and has served as adjunct nursing faculty at Cumberland University, allowing her to share her clinical experience with future generations of nurses.

Today, Stormiee brings decades of healthcare experience to her patients and has a particular passion for caring for older adults and medically complex patients. By bringing primary care directly into assisted living and skilled nursing settings, she helps provide continuity of care for patients who may otherwise face challenges accessing routine medical services.
