Dr. Roger McKinney received his B.S. degree from Middle Tennessee State University and went on to receive his M.D. degree from the University of Tennessee Center for Health Services in Memphis, Tennessee. He completed his residency in Family Practice at the University of Alabama in Huntsville and then moved to Lebanon in 1980, where he has been the co-owner of Family Medical Associates ever since.Dr. McKinney is a member of the American Academy of Family Practice, the Wilson County Medical Society, the Tennessee Medical Association, and the American Society for Psycho prophylaxis in Obstetrics. Dr. McKinney lives in Lebanon and enjoys showing horses and pleasure riding, motorcycling, snow skiing, and gardening.

Dr. Bernie Sy graduated from Vanderbilt University then went on to Ole Miss for Medical School. He completed a combined residency in Internal Medicine and Pediatrics with training at East North Carolina Medical Center where he was an esteemed chief resident and was honored being named "Resident of the Year".Originally from Louisiana, Dr. Bernie Sy joined Family Medical Associates in 2007. He resides in Lebanon with his wife, Christine and four children Noah, Olivia, Benjamin, and Gabriella. We are excited to have him as the newest member of our team.

Dr. Crutcher earned his B.A. in Biology, graduating magna cum laude from Lipscomb University in Nashville, where he was president of the student government association and in the Alpha Chi Honor Society. He earned his M.D. from the University of Tennessee Health Science Center, College of Medicine, Memphis, and completed his residency program at Palmetto Health Richland in Columbia, S.C., where he received the Most Compassionate Resident Award, Resident Teacher of the Year Award and served as Resident Council President. Dr. Crutcher completed a fellowship in healthcare quality improvement while serving as Clinical Assistant Professor at the University of South Carolina, School of Medicine, Department of Family and Preventive Medicine.
Dr. Crutcher is board-certified and a member of the American Academy of Family Physicians and Society of Teachers of Family Medicine. He received recognition from the National Committee for Quality Assurance (NCQA) for providing quality care to diabetic patients in the NCQA Diabetes Recognition Program.
As a native of Lebanon, we are proud to have him as a co-owner of the Family Medical Team!

Dr. Bill Robertson graduated from David Lipscomb University with a degree in Biology and went on to obtain his medical degree from the University of Tennessee, Memphis College of Medicine. He completed a combined residency in Internal Medicine and Pediatrics with training at Bowld University Hospital, the Regional Medical Center, Veterans Administration Hospital, Baptist Memorial Hospital, and LeBonheur Children's Medical Center.
A native of Lebanon, "Dr. Bill" is the son of Dr. George Robertson. He joined his father in practice with Family Medical Associates in 2001. He resides in Lebanon with his wife, Michelle, also a Lebanon native, and their 4 children, Rebecca, William, James, and Noah.

Benita Qualls, PA received her B.S. degree as well as her Physician Assistant Certification from Trevecca University in Nashville, Tennessee. She has been a Physician Assistant with Family Medical Associates since 1995. Benita currently lives in Murfreesboro with her husband Dan and their four children Bethany, Kaitlyn, Daniel, and Jordan.

Stormiee worked at Vanderbilt Children’s Hospital in the capacity of a Registered Respiratory Therapist for nine years after receiving her BS degree in Cardio-Pulmonary Science from Tennessee State University. While working in the ER, Pediatric Intensive Care and Neonatal Intensive Care she soon desired advancement of her studies. That enlightened her to the nursing field and after achieving her BSN from Cumberland University, she was able to transition to a Pediatric Intensive Care bedside nursing role with a strong focus in cardiac recovery. After completion of her Masters Degree in 2010 and completion of ANCC board certification she began a career which enabled her to have a diverse background in family medicine, with a rural health focus and now geriatrics with the focus being in long term and skilled care management. This has been my primary focus and passion for the last five years. She has been blessed with an extensive and diverse medical career that has extended over 24 years and a wonderful family. Currently, Stormiee resides in Smith county with her husband and two children.

Richard Kincaid, FNP obtained his BSN from Cumberland University and his MSN from Tennessee State University. He completed a post graduate certificate in nursing education and is a certified nurse practitioner. He is currently an adjunct faculty at the nursing program at Cumberland University. Prior to joining the family medical team in 2009, Richard worked as a research coordinator and an ICU nurse at St. Thomas Hospital.
Richard moved here from Spain in 1996 and is fluent in both Spanish and English. He is now married with 2 children, residing in Lebanon.

Dr. Jim Reed earned his B.A. in Arts at the University of Tennessee in Chattanooga. In May 2002, he pursued his Medicine Degree at West Virginia School of Osteopathic Medicine.

Kelly Louvin, PA completed her B.S. in chemistry at Tennessee Tech University and went on to attend the Physician Assistant program at Lincoln Memorial University. She spent 4 years working in a pediatric practice in Chattanooga after graduation. Kelly joined the Family Medical team in 2021.

Carrie Forhetz, PA-C received her B.S. degree in Biology at Freed-Hardeman University, graduating summa cum laude. She then went on to further her education and pursue medicine at Lipscomb University’s Physician Assistant Program where she received her Master of Science in Physician Assistant Studies and was inducted into the Phi Alpha Honor Society for physician assistants. Carrie is originally from West Tennessee.
She is married and currently resides in Nashville, TN with her husband, Evan, who is also a physician assistant and a Nashville native. In her free time, Carrie enjoys exercising, hiking, traveling, skiing, and spending time with her husband and family.

Adelia Birdwell, PA-C attended Lipscomb University where she obtained her B.S. in biology with a minor in both chemistry and psychology. She was involved in student government as a class senator all four years and Greek life on campus. After her undergraduate degree, she stayed at Lipscomb University and earned her masters of physician assistant studies.
Adelia is originally from Cookeville, TN, but currently resides in Nashville. She enjoys being outdoors, traveling, and spending time with the people she loves.
